Financial assistance programs for patients with cancer, kidney, liver cirrhosis, paralyzed stroke, congenital heart disease and thalassemia.

The Financial Assistance Program for Patients with Cancer, Kidney, Liver Cirrhosis, Paralyzed Stroke, Congenital Heart Disease and Thalassemia has been launched from the financial year 2013-14. The work is being carried out by identifying the poor patients suffering from the disease, compiling the list of real destitute and helpless persons by following the policy in collaboration with the manpower of the Department of Social Services, local administration, people's representatives and well-wishers. Poor patients suffering from Cancer, Kidney, Liver Cirrhosis, Stroke Paralyzed, Congenital Heart Disease and Thalassemia are provided medical assistance at the rate of Rs. 50,000 / - (fifty thousand) per person. This helps the affected family to bear the expenses of the family and return to a healthy and normal life.

Please read the following guidelines carefully before applying:

A. The applicant / patient must be a citizen of Bangladesh; The highest distressed person will be given priority; In case of financial situation: Children, the destitute, refugees and landless will be given priority; In case of social status: priority will be given to the elderly, widows, divorced, widowed, destitute, separated from family; Preference will be given to landless candidates where applicable; In this case, if the amount of land of a person other than the homestead is 0.50 acres or less, he will be considered landless.

B. The complete application must be submitted to the concerned Upazila Social Service Office by attaching the following certificate, prescription / test report and information with the application:

1. Original copy of the relevant disease certificate issued by a registered specialist doctor should be attached in the prescribed form;

2. Patients suffering from Cancer, Kidney, Liver Cirrhosis, Stroke Paralyzed, Congenital Heart Disease and Thalassemia must submit the following prescription and test report along with the application to the specialist doctor in the respective disease:

2. 1. Histopathology / Cytopahtology / Bone Marrow Report or other test report in case of cancer;

2. 2. In the case of Kidney Disease, reports of patients receiving Acute Renal Failure or Chronic Renal Failure, receiving dialysis services, preparing for a kidney transplant or having a kidney transplant, and reports of urea and creatine levels in the blood;

2. 3. Liver cirrhosis should include liver ultrasonogram report and other test reports;

2. 4. Patients with paralyzed stroke must be certified by a neurologist and MRI / CT Scan Report;

2. 5. Congenital heart disease should include Echo Cardiogram report and other test reports;

In case of thalassemia, there should be a report of hemoglobin electrophoresis or other applicable tests in the blood.

3. Attested photocopy of the patient's National Identity Card / Birth Certificate by 1st Class Gazetted Officer and 3 newly taken attested photograph;

4. If the patient is under 18 years of age / dead at the time of application, his / her legal guardian can apply, in which case a photocopy of the applicant's national identity card attested by the 1st class gazetted officer; And 3 recent attested photos.
